Because the Writers just thought of it.
My guess is that he was getting tired of hanging around Twilight and made the whole thing up.
Those past events lead to the creation of his code.
Because he hadn’t made it up yet.

definitly this wasnt an absolutly and undeniably horible chapter…still it did feelt a bit of considering spike its pretty grown up at least on the personality department, he has showed to be reliable and smart, so it did felt off that spike sudendly couldnt do anything good, regardless it was an interesting one, not my particular favorite but not completly bad
Because Dave Polsky came up with a shitty concept for the story, and Merriwhether just had to roll with it.

I’d assume it’s because he only just thought it up recently. When Twilight mentioned “Dragon Code”, she probably meant that all dragons make up their own strict moral code.

In my head i can see him basically cribbing the whole code from stuff he’s read but not necessarily understood. Like maybe dragons do have some honor code thing going on but spike’s sources are missing most of the details, are full of guesswork or are just plain wrong.

But I also want to believe that’s something the teenage dragons taught him, and he has been reflecting on it ever since, and by the time of “spike at your service” he had decided to follow that code in order to mature/act older/etc, etc.

I took it as he created it sometime after Dragon Quest. Like after that episode he decided that he didn’t want to turn his back on being a dragon completely, so invented his own well meaning but flawed code of conduct in the hopes of becoming the world’s first noble non-asshole dragon.
 
He calls it “Spike’s dragon code” specifically. I always assumed it as something he himself made up
