Probably just no public restrooms. Ponyville’s been said to have a public water system to the homes (Spike’s showers, Fluttershy’s + CAkes’ bathrooms), so it would be expected they’d have facilities in homes too. But some place like Dodge, there’s probably no running water, so a communal facility is common.
And for here, the common male/female iconographs that we use for restroom IDs don’t have easy pony equivalents (its based on the concept of a dress) so Twilight may have figured it was a restroom but no idea that it was gender-based.

Okay, wait. Why would a place like Dodge Junction have restrooms whereas a more advanced place like Ponyville wouldn’t? They’d at least have the same kind of restrooms as Dodge Junction.

that wasn’t Ponyville, that was Dodge Junction. as far as we know, Ponyville is bathroomless.
they DO have outhouses, though.
