If you train your spine to be flexible then it’s possible to do the pose in the OP. I can actually do that pose myself if I have a wall as . The pose in the picture you gave has exaggerated/mismatched anatomy but it is possible to twist and bend the spine in that way if the person has trained their muscles to stretch in a way that allows the spine to move.
